Healthcare Overview: New Perspectives

Volume Editor: Vincenzo Costigliola, Brussels, Belgium Just a few years ago, dire predictions of escalating technology costs and adverse demographics informed a downbeat view of the future of medicine in which struggling governments would fall behind in their public commitments to healthcare. This volume is part of today s more optimistic assessment in which huge efficiencies can be wrought through personalised medicine: patients, for example, have no need to visit clinics when biochemical monitoring can be achieved in the comfort of their own homes, the results sent wirelessly, in real time, to the specialist s database. With contributions spanning the experiences of   52 authors from 15 nations from the USA to Turkey, Taiwan and the UK, the contents of this book show what is possible in implementing personalised medicine in a heterogeneous set of contexts. Today s burgeoning use of advanced early and predictive diagnostics, targeted prevention and personalised medical approaches will accelerate as advances in patient profiling and innovative bio/medical technologies come on stream. They could enable many of tomorrow s elderly to reach 100 years of age in good physical and mental health, as actively contributing members of society. The scope of coverage in this volume provides a comprehensive and unrivaled review of historical, cultural, demographic, ethnic, socio-economic, political, and other aspects of personalised medicine. It shows that reaping the benefits of this kind of medical care requires careful regulations in healthcare and skilful political planning. Links Springer Link

New Strategies to Advance Pre/Diabetes Care: Integrative Approach by PPPM

Volume Editor: Mahmood S. Mozaffari, Augusta, GA, USA The writings of earliest civilisations reveal that diabetes mellitus has affl icted man for a very long time. Unfortunately, however, we are now experiencing a worldwide pandemic of the disease which has not been adequately addressed. Available evidence suggests that worldwide every 10 seconds one patient dies of diabetes and its severe complications. The outlook is even more bleak with the prediction of half of a billion diabetic patients by the year 2030. Th e cost-impact of the disease poses a major challenge for both developed and developing countries and their economies. Thus, it is high time to reconsider and revise the current strategies in diabetes care. Accordingly, a paradigm shift from reactive to perspective medicine has become a timely consideration. Th is shift is based on the concept of Predictive, Preventive and Personalised Medicine (PPPM). The rationale for PPPM relates to the recognition that prediction of persons at high risk for the disease (i.e., diabetes mellitus) should help devise strategies to prevent its target organ complications, and create eff ective treatments tailored to the individual patient, thereby reducing morbidity, and associated costs, and mortality. The current book entitled “New Strategies to Advance Pre/Diabetes Care: Integrative Approach by PPPM” is intended to serve as a reference source for researchers and the healthcare industry with a special emphasis on health promotion in the general population and help design innovative strategies to prevent the onset of diabetes mellitus and its secondary complications. Neurodegenerative Diseases: Integrative PPPM Approach as the Medicine of the Future

Volume Editor: Silvia Mandel, Haifa, Israel The economic burden of dementia was estimated as being US$ 604 billion in 2010. Now it is time for new guidelines in handling of neurodegenerative disorders. Optimistic versus pessimistic prognosis depends much on diagnostic and treatment approaches which healthcare will preferably adopt in the near future. Current dramatic situation can be signifi cantly improved by the implementation of appropriate early diagnostic, targeted preventive measures and therapies tailored to the person advancing healthcare and enhancing the patient life quality. Neurodegenerative diseases are pathologies of multifactorial nature involving interplay of epi/genetic and environmental risk factors. Insights into molecular pathomechanisms will enable to create the most eff ective neuroprotective strategies and individualised treatment before manifest pathology. Utilisation of the advanced integrative medical approach could enable a significant portion of population to reach the 100-year age limit remaining vibrant in excellent physical and mental health as actively contributing members of the society. The crucial role of the integrative approach by PPPM has been acknowledged as priorities by World Health Organisation, United Nations and the European Union among others. Extensive eff orts of international consortia are essential in the fi eld to consolidate the accumulated expertise and to create population screening, robust diagnostic approaches, eff ective prevention in groups at high risk and targeted treatments. This book volume “Neurodegenerative Diseases: Integrative PPPM Approach as the Medicine of the Future” is intended to serve as a reference source for basic researchers and healthcare industry involved in the fi eld of neurosciences with a special emphasis on mental health promotion and innovations intended to delay or even prevent neurodegenerative diseases. New Strategies to Advance Pre/Diabetes Care: Integrative Approach by PPPM

Volume Editor: Jorge Coelho, Coimbra, Portugal This book is part of a series dedicated to recent advances on predictive, preventive and personalised medicine (PPPM). It focuses on the theme of “Drug delivery systems: advanced technologies potentially applicable in personalised treatments”. The critical topics involving the development and preparation of eff ective drug delivery systems, such as: polymers available, self-assembly, nanotechnology, pharmaceutical formulations, three dimensional structures, molecular modelling, tailor-made solutions and technological tendencies, are carefully discussed. The understanding of these areas constitutes a paramount route to establish personalised and eff ective solutions for specific diseases and individuals. Links Springer Link

Rare Diseases – Integrative PPPM Approach as the Medicine of the Future

Volume Editor: Meral Özgüç This book focuses on predictive, preventative and personalized medicine (PPPM) and how it is related to the healthcare of rare diseases. Readers will discover how advanced rare diseases healthcare provides an excellent “proof-of-principles” for the personalisation of healthcare systems on a global scale. Chapters look at national plans for rare disease, at biobanking, gene identification, rare cancers, virus gene therapy , induced pluripotency for cell therapy amongst   other topics. There is a chapter dedicated to personalized medicine for hereditary deafness, and another exploring the complexity of genotype-phenotype correlations. Specific diseases such as Fabry’s, Gauchers and mitochondrial cytopathies are highlighted and we look at enzyme replacement therapy in lysosomal storage diseases. This work is part of a series, produced with the involvement of the European Association for Predictive, Preventive and Personalised Medicine. The series focusses on the concept of an integrative medical approach by PPPM. This volume is dedicated to all aspects related to the prediction, prevention and personalised treatments of rare diseases, and in doing so it explores developments relevant to all medical branches. The authors cover ethical considerations, the creation of a robust platform for professional communication, synergies with patient organisations, “doctor-patient” collaboration and a new philosophy of integrative medicine by PPPM. This volume serves as a reference source for scientific and medical centres in the field and can be used both at medical curricula and graduate level in the life sciences. Those who place a special emphasis on healthcare promotion and innovations intended to combat rare diseases, save the affected lives and enhance life quality will all find this book of great value. Individualized Medicine – Ethical, Economical and Historical Perspectives

Volume Editors: Tobias Fischer, Martin Langanke, Paul Marschall, Susanne Michl A comprehensive and well-grounded outline of Individualized Medicine Presentation and assessment of concrete examples based on multi-year research Intensive discussion of barriers for translation concept of Individualized Medicine into clinical routine In 2009 the University Medicine Greifswald launched the “Greifswald Approach to Individualized Medicine” (GANI_MED) to implement biomarker-based individualized diagnostic and therapeutic strategies in clinical settings. Individualized Medicine (IM) has led not only to controversies about its potentials, but also about its societal, ethical and health economic implications. This anthology focusses on these areas and includes -next to clinical examples illustrating how the integrated analysis of biomarkers leads to significant improvement of therapeutic outcomes for a subgroup of patients -chapters about the definition, history and epistemology of IM. Additionally there is a focus on conceptual philosophical questions as well as challenges for applied research ethics (informed consent process, the IT-based consent management and the handling of incidental findings). Finally it pays attention to health economic aspects.   The possibilities of IM to initiate a paradigm shift in the German health care provision are investigated. Furthermore, it is asked whether the G-DRG system is ready for the implementation of such approaches into clinical routine. Links Springer Link

An Information Technology Framework for Predictive, Preventive and Personalised Medicine: A Use-Case with Hepatocellular Carcinoma

Volume Editors: Leonard Berliner, Heinz U. Lemke Provides a roadmap for integrating Information Technology with PPPM Proposes a methodology for managing vast amounts of personal medical data for decision support by means of Bayesian networks Introduces new concepts such as a Digital Patient Model and Model Guided Therapy This book explores how PPPM, clinical practice, and basic research could be best served by information technology (IT). A use-case was developed for hepatocellular carcinoma (HCC). The subject was approached with four interrelated tasks: (1) review of clinical practices relating to HCC; (2) propose an IT system relating to HCC, including clinical decision support and research needs; (3) determine how a clinical liver cancer center can contribute; and, (4) examine the enhancements and impact that the first three tasks will have on the management of HCC. An IT System for Personalized Medicine (ITS-PM) for HCC will provide the means to identify and determine the relative value of the wide number of variables, including clinical assessment of the patient — functional status, liver function, degree of cirrhosis, and comorbidities; tumor biology, at a molecular, genetic and anatomic level; tumor burden and individual patient response; medical and operative treatments and their outcomes.
